General Required Meeting and Entertainment (M&E) Backup documents:

  1. Completed AFO M&E Form
    • For the Excel version, contact AFO member directly.
    • Note: This form requires an attendee list.
    • If P-card used:
      • P-card M&E Form additionally required.
      • Simon’s signature needed on P-card M&E Form if exceptional.
  2. Itemized receipt/invoice
  3. Invitation
  4. Agenda (or discussion topics)
  5. For Multiple Event Expenses:


IMPORTANT:

  • If any of these 5 items are missing, please provide further explanation in the Business Justification via the AFO M&E Form.
  • All morale events are considered exceptional and require SOM Dean’s Office exceptional approval.
  • All materials must be submitted as PDF files with the contents facing the right way (not sideways).
