From your first day at UCSF to your very last day here, you will be covered by our Professional Liability (PL) insurance, aka malpractice insurance.

When you are renewing your credentials or applying for a new job outside of UCSF, you will be asked for your proof of PL insurance coverage and a claim history (loss run report). Sometimes the credentialing specialist may ask you to get this information, our internal website explains how to do this.

Halfway down the internal website page is a link titled “UCSF Health Authorization to Release Claims Hx & PL Verification PDF Form.” Please fill out the form and follow steps #1 through #3.

As a UCSF employee working at a non-UCSF location, you will need evidence  of your UCSF malpractice coverage, this is provided through a COI.  Note: Work done outside of UCSF must be done pursuant to a Professional Services Agreement (PSA).

NOTE: COIs for Anesthesia faculty can be requested by contacting Joanna Times.

As a physician/practitioner here at UCSF you may receive a subpoena for deposition testimony, to appear at a Civil or Criminal Trial, or Hearing. UCSF Risk Management accepts service of subpoenas for personal appearance ONLY as a courtesy to you. If you do not want Risk Management to accept on your behalf, please let us know and we will direct process servers to locate you. We do not accept other legal documents on your behalf. Regarding the public website, you can refer process servers and lawyers to it. We highly recommend that you become familiar with them.

From your first day at UCSF to your very last, you are covered by our Professional Liability (PL) insurance, aka malpractice insurance. Coverage is occurrence based and there is no need for Tail coverage upon your departure. Over the years, when applying for other jobs or being recredentialed, you may need to provide this information to your new institution’s medical staff office. Below is where you can find information about it, please refer to the Professional Liability Section - not the section on COIs.

When you are recredentialing or applying for a new job, your proof of PL insurance coverage and claim history (loss run report) will be requested. Sometimes the credentialing specialist may ask you to get this information. Please go to #10 on the website page and follow the instructions for assistance, and/or you can share this website link with the credentialing specialist.

After leaving UCSF, you may be served a Summons and Complaint for a claim that happened during your time here. Please contact UCSF Risk Management as soon as possible at [email protected].
