Pirracchio Data Science Lab

Home

The “Pirracchio Lab” is a data science lab focusing on the development and the use of innovative and disruptive statistical methods to analyze complex data arising from various acute care settings including (but not limited to) trauma, anesthesia and critical care. In this very competitive environment, our particularities are i) to always focus on research questions arising from the clinical ground; ii) to take a comprehensive and translational approach ranging from theoretical mathematical development, data collection and processing, down to real-life evaluation of the predictive / prescriptive tools. In this endeavor, we are closely collaborating with the division of Biostatistics at UC Berkeley, the Department of Applied Mathematics the Department of Biostatistics at the University of Paris.

I am a French M.D., Ph.D., hailing from Paris. I obtained my M.D. in 2003, with a specialization in Anesthesiology and Critical Care Medicine. I completed my medical training by a 2-year fellowship in Critical Care Medicine. In 2008, I obtained a Master degree in Medical Research Methodology and Biostatistics. I completed my doctoral studies in the Department of Biostatistics and Medical Informatics (DBIM, INSERM U-1153) at Hôpital Saint Louis, Paris, France in 2012 under the guidance of Prof Sylvie Chevret. In 2012-2013, I spent a year as a postdoctoral fellow in Biostatistics in the School of Public Health at the University of California, Berkeley, where I worked under the supervision of Prof. Mark J. van der Laan and Prof Maya L. Petersen. Back in Paris, I was the clinical director of the surgical and trauma ICU at European Hospital Geroges Pompidou (2013-2015) and a researcher in Biostatistics at the INSERM U-1153 unit. In January 2015, I have joined the Department of Anesthesia and Perioperative care at the San Francisco General Hospital & Trauma Center (UCSF) as Associate Professor. In September 2016, I came back at European Hospital Geroges Pompidou in Paris where I served as Full Professor and vice chair for the ICUsand then Chairman of the Department of Anesthesia and Critical Care Medicine. Since September 2018, I joined UCSF, where I am serving as Chief of Anesthesia and Perioperative Medicine at Zuckerberg San Francisco General Hospital and Trauma Center and Vice Chair of the Department of Anesthesia and Perioperative Care.

My two main research areas are clinical research in Anesthesiology/Critical care Medicine and applied research in Biostatistics.

Clinical Research

I have three particular focuses in clinical research: evaluation of cardiovascular life support therapies (ECMO, ECLS); optimization of critical care for trauma patients; evaluation of the prognosis of patients in anesthesiology and/or critical care.

Research in Biostatistics

I am broadly interested in problems of causal inference and predictive and prescriptive analytics. In particular, I enjoy working on the development of novel methodologies for addressing scientific questions using big medical data and complex observational data subject to sampling biases.

Research

We are broadly interested in problems of prediction and causal inference. In particular, we enjoy working on the development of novel methodologies for addressing scientific questions using complex observational data subject to sampling biases.

Machine Learning, Prediction and Personalized Medicine in the ICU

Predicting the risk of death is crucial in Critical Care as it allows stratifying the both patients for clinical or research purposes.  We realized that the poor performances of the score currently used could be due to the fact that they roughly all rely on parametric models. We work on optimizing the ICU mortality prediction tools by using innovative machine learning methods, such as the Super Learner.  Based on such methods, we have been able to propose a new prediction algorithm that clearly outperforms the SAPS II or the SOFA scores.

We are also interested in harnessing Big Data generated from continuous ICU monitoring by leveraging upcoming innovation in online Machine Learning to ultimately be able to provide personalized real-time prediction and decision tools that would help the clinician in the ICU. We have recently created together with Prof. Antoine Chambaz an international research team dedicated to this (ACTERREA) which was officially labelled by the French National Research Agency (CNRS). Because we acknowledge that predicting mortality may not be perceived as clinically useful by the doctors, our goal is to produce highly efficient predictive tools to predictive different sorts of relevant intermediate outcomes in critically ill patients. By relevant intermediate outcomes we mean outcomes that are i) frequently observed; ii) highly correlated to mortality ; iii) actionable, i.e., potentially preventable or at least treatable. In this vein, we are currently working on a new generation of hypotension predictors that have the ability to harness time-series, learn in real time and thus produce personalized prediction.

We are also working on prescriptive analytics with the ultimate goal to develop clinical decision support based on AI. For instance, we have developed a machine learning based classifier that can help the paramedics to choose whether a trauma patient should be transferred or not to a level-I trauma center.

Model Interpretability

The lack of buy in for machine learning / AI can be explained by the “black box” nature of the algorithms. We are working on 2 different approaches to tackle this problem : i) variable importance measure; ii) including expert matter knowledge.

The problem of variable importance measure is crucial in Anesthesiology and Critical Care, in order to provide to clinicians some useful information concerning the variables they should care about for their patients. Knowing more about the variables that mostly drive the prediction helps to improve model interpretability. We are working on applying methods coming from the causal inference world to this question. Specifically, the use of collaborative targeted maximum likelihood estimation (cTMLE) seems to offer some great advantages in the context of high dimensional sparse observational data.

Including expert knowledge is also of paramount importance when training machine learning algorithms. We are working on several projects in this area. One focusses on revisiting the expert guidelines for traumatic brain injury and hemorrhagic shock to determine the minimal bundle of actions that the clinician should take to optimize the outcome. The other one, in collaboration with Dr Gilmer Valdes, aims to augment a random forest classifier for ICU mortality by incorporating physician knowledge in the learning process.

Causal Inference Propensity Score Analyses

We have addressed many problems related to observational data, trying to draw causal relationship between an exposure and an outcome. We naturally applied several very popular propensity score approaches and evaluated the performances of these methods in different settings. However, several questions have risen when using these methods in practice, essentially regarding the form of the propensity score model, and the choice of the variables to be included in this model. Hence, one of our goals is to take advantage from the great advances in machine learning in order to improve the performance of propensity-based estimators. For instance, we have developed specific evolutions of the Super Learner, a method derived from the stacking algorithm proposed by A. Hubbard and M. van der Laan, to model the propensity score.

Causal Inference for Longitudinal Data Structures

We were faced with the clinical question of estimating from observational data the benefit of ICU admission in patients with multiple successive triages. This question is typically a problem of causal inference for multiple time-point data. Our contribution to this specific question has been to evaluate different statistical approaches for this specific problem, based on marginal structural models on the one hand, on instrumental variables on the other hand.
